Please print the necessary player registration forms for your Nevada Elite team, Registration Instructions, Player-Parent Agreement, GBYSL Conduct Form, Scholarship Form, USYS Membership Form. This includes all forms necessary for players to register to play on a Nevada Elite FC team, and must be completed by each player prior to playing with their team.  It includes a scholarship application for those players who desire to apply for financial assistance.
